• An Eldercare Service Representative is the primary day-to-day contact and central communications point for both the family and their loved one.
  
 
• A Social Worker is the pivotal link between Freedom Eldercare, the in-home caregiver and client. Freedom Eldercare is the only home health care company that offers this complimentary service to ensure quality care and adherence to the care plan for your loved one. The social worker addresses, mediates and offers solutions to crisis situations, as well as teaches the caregiver behavioral skills.
   
  
• A Registered Nurse develops, modifies and oversees the customized care plan and supervises the caregiver for your loved one. Each individual care plan outlines the appropriate combination of elder care personnel and home health care services that will help keep your loved one happy and safe.
   
  
• A Caregiver is screened and carefully selected to address the individual needs of the client, whether he or she is living at home or in another residential facility. The caregiver may be a Certified Home Health Aide, Certified Nursing Assistant or in-home companion.
